Programs Offered
- Diabetes Prevention Program (DPP) 
        For adults with prediabetes. - Proven to stop/delay progression to type 2 diabetes. 
- Weekly/bi-weekly sessions for 6 months, then monthly for the rest of the year.
-  In-person or Zoom. 
 
- Intensive Diabetes Self-Management Education and Support (iDSMES) 
        For adults with type 2 diabetes. -  Improves knowledge, skills, and self-efficacy. 
- Weekly/bi-weekly sessions for 6 weeks, then monthly for the rest of the year. 
- In-person or Zoom. 
 
- Chronic Disease Self-Management Program (CDSMP)- For people 60+ and caregivers with chronic conditions. 
- Weekly 2.5-hour sessions for 6 weeks. 
- In-person or live online. 
 
- Diabetes Self-Management Program (DSMP) 
        For people 60+ with diabetes and caregivers. - Weekly 2.5-hour sessions for 6 weeks. 
- In-person or live online. 
 
- Toolkit for Active Living with Chronic Conditions - For people 60+ with chronic diseases and caregivers. 
- Toolkit by mail + weekly 1-hour phone chats for 6 weeks. 
 
- Health Coaches for Hypertension Control (HCHC) 
        For people 60+ with hypertension and caregivers. - Weekly 1.5-hour sessions for 8 weeks. 
- Free electronic blood pressure monitor included. 
- In-person or live online. 
 
- Geri-Fit®
 - Strength-building program for people 60+ and caregivers. 
- Twice weekly 45-minute sessions for 12 weeks.
- In-person or live online. 
 
- Walk With Ease (WWE) 
- For people 60+ with arthritis and caregivers.
- Reduces pain, improves balance and strength. 
- Thrice weekly 45-minute sessions for 6 weeks or self-paced with periodic check-ins.
-  In-person or book-based.
